Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. In preparing my translation I have had the advantage of the co-operation of Mr. James Speirs, until his sudden and untimely death, and the invaluable assistance of my friend, Dr. T. Miller Neatby, m.a., who has constantly criticized the work both as physician and as writer, giving a value to this version which it would otherwise have lacked.
